Youth to Adult Program (YAP) is a transition planning project that helps youth and young adults (14-25) and their families navigate the shift from adolescence to adulthood.
The Youth Adult Partnership (YAP) serves as a vital compass for youth and young adults aged 14 to 25, as well as their families, during the often-complex transition from adolescence to adulthood. This pivotal life stage involves navigating a shift in services, expectations, and personal independence that can feel overwhelming without a dedicated roadmap. By providing a structured framework for transition planning, YAP ensures that young people aren't just reacting to changes, but are actively steered toward a future that aligns with their personal goals and needs.
The core of the program focuses on a comprehensive assessment of where each individual currently stands, paired with the collaborative creation of a long-term vision. We work closely with participants to identify their unique strengths and interests, helping them define exactly where they want to go in life. Once that destination is clear, YAP assists in developing a concrete, actionable plan to get there, breaking down large, intimidating milestones into manageable steps that build confidence and momentum.
Beyond high-level planning, YAP provides practical, "boots-on-the-ground" support to ensure participants are ready for the workforce. This includes hands-on assistance with essential job preparedness tasks, such as crafting professional resumes, mastering interview techniques through guided practice, and developing effective job search strategies. By equipping youth with these fundamental professional tools, YAP empowers them to successfully enter the community as prepared, self-assured, and independent adults.
